Only registered Healthcare Professionals who reside in New Zealand are eligible to enrol in the Connect 360 Patient Support Program (“Program”) and access the HCP Portal (“Portal”). This is a complimentary service offered by AstraZeneca Pty Ltd. The Program is administered by DKSH HEALTHCARE AUSTRALIA on behalf of the sponsor AstraZeneca Pty Ltd (‘AstraZeneca’). In completing your enrolment to participate in the Program, you consent to DKSH HEALTHCARE AUSTRALIA collecting and using the personal information you provide regarding yourself (e.g. name and contact details) to contact you in relation to the Program, including patient-related updates and otherwise, as necessary, to undertake the Program.
The Portal can only be accessed by secure login upon registration. Prescribing Specialists can login to the Portal to access: an online tool that allows Prescribing Specialists and their Nominated Users to access a Patient Progress Dashboard, which sets out the status of all Prescribing Specialist’s patients who have been linked to their Account including any post-injection reports from their nurse, their patient’s previous and next injection date, and status. The Patient Progress Dashboard is provided to assist you with managing your patients, but you should not rely solely on this for managing your patients.
During the registration process, you have the opportunity to provide details for a Nominated User to access the Patient Progress Dashboard. The Prescribing Specialist confirms that the Nominated User is authorised to be linked to the Patient. The name of a Prescribing Specialist and his/her Nominated Users, their contact details, total number of patient enrolments, and channel of enrolment will be collated into a report for AstraZeneca. Patient-specific information will not be included in any report provided to AstraZeneca. However, AstraZeneca may use this information for internal purposes, including planning and managing resources for the Portal, and analysis and research purposes (e.g. to calculate patient adherence rates). AstraZeneca may disclose the information to third parties in order to promote awareness of the Portal and to publish the information in reports or medical publications. The name of the Prescribing Specialist and his/her Nominated Users will not be able to be identified in any form of report or publication. Astra Zeneca and DKSH Healthcare will always comply with their Privacy obligations in accordance with the Australian Privacy Act 1988 (Cth) and the New Zealand Privacy Principles contained in the Privacy Act 2020 (NZ) (“the Principles”) and ("the Privacy Acts") as applicable. In New Zealand, to the extent that personal information is health information, AstraZeneca is also bound by the New Zealand Health Information Privacy Code 1994 ("Health Code").
All of the services offered as part of the Program are optional allowing you to opt into all or just a selection of the services provided. You may opt-out of any part of this program at any time by contacting the Connect 360 Clinical Liaison support@connect360asthma.co.nz. If you opt-out of this Program, we may still need to hold the information collected from you as part of the Program for purposes permitted by law including reporting purposes. To the extent we retain any personal information, it will be handled in accordance with the Program Privacy Policy.
AstraZeneca reserves the right to cease funding the Program and/or terminate the Program with reasonable notice at any time. AstraZeneca reserves the right to change and/or cease providing any of the services within the Program with reasonable notice at any time.
